Interview Score Sheet and Questions

The following model is provided for your use. You may add, delete or modify questions as appropriate for the specific situation but any changes must be made prior to a copy being distributed to the designers being interviewed.

Prior to the Interviews, the client should determine the "weight" that is to be assigned to each of the items being evaluated with the range being from a high of 10 to a low of 1. This helps assure that all "Interviewers" are basing their evaluation using the same weight. The "weight" should be an indication of the relative importance the client places on each item being rated. Each Interviewer should rate Designers independently. Ratings should be indicated on a 1 to 5 scale with 5 being the highest rating
